Name the things that your team should stop doing. This can be an overly-pricey process. Practices that are ineffective or that bring a negative cultural impact.
List the items that your team should continue practicing.
As a team, you should agree in advance if feedback should be focused around a specific topic (For example: Code review practices) or broad, reaching any aspect of team life.
As with any retrospective, a scrum master or a facilitator must make sure that participants stay within a constructive discussion that is aimed at improving the experience for the team. No format can turn a blaming session into a productive discussion.
